Yes. Alongside the medical standard, the credit requires that you retired on disability and receive taxable disability income from an employer's plan. Social Security disability payments on their own do not count as employer disability income, so it's worth checking what you receive before you apply.

Usually not. The Schedule R instructions contain a specific statement with specific signature lines, and a general letter confirming a disability does not satisfy it. This is the single most common reason a claim gets held up.

It depends on which line your provider signs. If they can confirm there is no reasonable probability your condition will improve, one statement covers you for every year afterwards. Otherwise you'll need a current one each time you file.

Anyone 65 or older, since age alone qualifies you for the credit. Veterans already certified by the VA on Form 21-0172. Anyone who already holds a statement signed on the no-improvement line. The credit also has low income limits and is nonrefundable, so it does nothing for you if your income is above the threshold or you owe no federal income tax. We'll tell you before you pay if any of this applies to you.
